After a big fight in Aunt Clarice wake, George is at the airport in Detroit trying to get a refund by showing the death certificate.
George says to the Airline Ticket Teller #2 the doctor that was attending Aunt Clarice suffered an untimely stroke, and lost the use of his right hand, so he was unable to write the death certificate.
Instead, George reaches inside his coat and takes out a Polaroid photo that shows him next to the coffin.
